    If you would like to roll over funds from a former employer’s 401(k) into your GalaxyOne Traditional IRA or Roth IRA (custodied by DriveWealth) we are happy to guide you through the process. 


    Step 1: Complete the Retirement Rollover Request Form

    Please complete the Retirement Rollover Request Form in full (attached below):

    Be sure to:

    • Indicate your DriveWealth IRA or Roth IRA account information

    • Provide details of your 401(k) plan administrator in Section 2

    • Select Option D as the transfer method in Section 3. Your current firm will mail a check to DriveWealth using the instructions provided on the form

    • Sign and date the Certification section


    Step 2: Provide a Recent 401(k) Account Statement

    Along with the completed form, you must provide a copy of your most recent 401(k) account statement

    This allows the rollover request to be reviewed and approved. The form must be countersigned by DriveWealth before it can be provided to your plan administrator.


    Step 3: Submit for Review and Countersignature

    Please submit the following to the GalaxyOne Client Services team:

    • The signed Retirement Rollover Request Form

    • Your recent 401(k) account statement

    Our team will review your documents and coordinate countersignature by DriveWealth on the Letter of Acceptance included in the form.

    Once fully executed, you may provide the signed paperwork to your 401(k) plan administrator to initiate the rollover.


    How Will the Funds Be Sent?

    Direct Check from Plan Administrator. You will instruct your 401(k) administrator to issue a check payable to:

    DriveWealth, LLC as Custodian FBO [Your Name]

    The check must:

    • Include your DriveWealth IRA account number in the memo line

    • Be mailed directly to the address listed in the form

    Important:

    • The check should not be made payable to you personally

    • Once the check has been mailed, please provide us with the tracking number so we can monitor delivery and assist if needed
